Atlantic Fellows for Health Equity Funding Request Form

The purpose of AFHE Collaboration Fund is to provide funding for projects that involve collaborations between Senior Fellows from the Atlantic Fellows for Health Equity program. We hope that the funds will help advance AFHE’s commitment to promoting innovative ideas, encouraging collective action and sharing the important work you are doing with others.


FUNDING CRITERIA


Project should involve 2-3 AFHE fellows in collaboration on a health equity focused initiative. 


  • Examples could include new community-based interventions or education, research endeavors, conference presentations. 
  • For conference presentations, you must provide documentation of an accepted abstract/poster/presentation that names each fellow as an author/contributor.

PROPOSAL PROCESS


Proposals will be accepted on a rolling basis. The proposal should be concise, approximately 1-2 pages and consist of the following elements:


  • Brief Narrative. Please describe the scope of the problem being addressed, the significance of your proposed project, the aims of the project, the specific approach you will take and why your idea is innovative.
  • Budget. This should include line items and a justification for budget line items. Please include other sources of support, if applicable. Note that funding from the fellowship may not cover all requested expenses.  
  • Deliverable(s). Please describe the concrete output of the funding.
  • Timeline.

Upon submission, please indicate your availability for a 30-minute check-in with the Senior Fellows Lead (afhe.seniorfellows@atlanticfellows.org) to discuss the proposal.


REPORTING


The fellows are expected to provide a copy of the deliverable(s) at the end of the project, within 60 calendar days of completion. This deliverable may be shared with the fellowship community through appropriate channels, e.g. newsletter, social media. We also encourage the fellows to try to disseminate their work on a broader basis, when feasible.

Fellows are also required to submit receipts or other equivalent documentation of use of funds at the end of the funding period. Any unused funds should be returned to AFHE.


RESTRICTIONS/LIMITATIONS


  • This is a one-time disbursement, not an ongoing funding opportunity. We cannot support continuing education requests. There is a finite amount of funding available each year, and there is no guarantee of approval or availability. 
  • Fellows may only receive one disbursement per calendar year.  The maximum disbursement is $5,000USD per project.  For conference attendance, support is limited to flight (booked via GW travel agency) and event registration fees.
